We would like to welcome you and your child to the Diagnostic Centre of Hořovice Hospital. We believe that you will be satisfied with the care and services of our health centre.
To make an appointment for a preventive check-up, vaccination or administrative tasks (health certificate, application for kindergarten, etc.), please contact the reception at 224 234 101 or by e-mail recepce@dcnh.cz.
In order to keep the office running smoothly and to maintain continuity of care, we ask that you call the office only for necessary matters (illness, issuance of an ID card).
Please have all the necessary information ready for your appointment:
- child's name;
- birth number;
- address, telephone, e-mail.
